Action Learning Mantra - Right or Wrong?

Revans said: There is no learning without action and no sober and deliberate action without learning.
Is this really true? Cannot some people keep taking the same action over and over without learning? Can organisations not do this as well when 'firefighting' on an almost daily basis?
Is this Revans mantra in fact correct?
