Who Initiates and Plans a Project?
It's very interesting to read about PMI processes, but I am wondering whether the initiating and the planning stages are carried out by top management (before choosing a project manager) or is it the role of a project manager to initiate and plan a project? Kindly explain, thanks.

Usually the top management viz., key stakeholders, sponsor, etc. would do initial activities like project charter which would include the business case, list of key stakeholders, high level constraints and assumptions, involving most essentially the project manager who would manage the project. PMBOK recommends involving the project manager as early as possible in the project phase, even initiating.
Once the project charter is approved by sponsor, then the real planning starts which is done by the project manager and his team. Activities in planning include requirements gathering leading to scope statement creation, scheduling, costing, and other plans related to hr, communications, risk, etc. It's utmost important to involve the team in detailed planning because the team needs to buy-in into the plans.
