Measuring per Person Productivity
I think it is very difficult to measure per person productivity in the world that we live in today where team effort is the primary model.
I think we should be focusing on the collective improvement in productivity and identifying the factors which slow down the team's productivity.
Measuring personal productivity serves no purpose because it will divert your attention from the systemic improvements which can be applied.

Low Productivity Mostly Likely not Caused by Low Individual Performance
@Vishal Vilas Bhalerao: In knowledge work areas (intangible products) the root cause of low productivity is very seldom low individial performance.
Most likely there are systemic causes (ineffective processes, ineffective organisational structures, local optimization (unit versus company goals), etc.
