To ensure these steady improvements take place, the concept of Kaizen can be used.

Collect all information, set the target and percentage of deviations, then implement the program. When the target can not be met, but still on the rule of percentage of deviations, that result is on exception.
When the implementation and result out of the rule percentage of deviation, that is mismanagement.
The management exception is the policy to accept the mismanagement, but not make risks and losses for the company.
