It is good to use 8D for problem solving. It resembles to DMAIC cycle used in six sigma. In fact DMAIC is very powerful way to solve problem compared to 8D.

Yes, there is a 5Ws opportunity. The 8D process is not limited to one form of identification. You may use any method appropriate to the situation, your level of comfort, and any other constraints (including regulatory). The same way you can employ any solution path, including DMAIC.

Wht the 5 Whys Should Be Utilised

A fishbone will not give you the ulitimate root cause of a problem, rather through consensus of the team 'most probable' causes will be identified. Once these are identified a 5 Why should take place until the TRUE root cause is identified, and then a solution can be sought and verified.
