This is not as straightforward as your question implies.
Information support is a "service system". To identify information needs, you need to start by understanding the "system served". So you would have to start by modelling "health" and using this model to derive information requirements.
Information sharing then becomes explicit through looking at responsibility for activities that depend on each other, and from identifying organisational boundaries. This is very well described in the book '
Soft Systems Methodology: Conceptual Model Building and Its Contribution'.
