What actions need to be taken to ensure successful 360 degree feedback? In what cases will it be a success? In what situations?
For starters, I think a 360 degree feedback system can successfully work where the PEOPLE PROVIDING FEEDBACK ARE MATURE enough to give the right feedback rather than for example focusing on the negative side of an individual.

360 feedback can be a very powerful experience, and in my view it's indeed important that this is a positive power, rather than a negative one. To create the right conditions for success my top four tips would be:
1. GO THROUGH THE PROFILE WITH THE CLIENT, rather than have them read through it beforehand. This could be face to face or via call/online chat. This helps to make sure they take the feedback in an appropriate way.
2. BE PREPARED. Make sure you are familiar with the profile beforehand so you can anticipate reactions to statements and scores.
3. Always TURN THE DISCUSSION INTO ACTION to end in a positive way. What can the client do to build on their strengths? What can they do differently where they are not making the impact they want?
4. And finally, BE RESPECTFUL. It's a privilege to be trusted to share such personal information about a client.

360 degree feedback is a great process for allround development of individuals.
I agree MATURITY IS KEY FOR ALL PARTICIPANTS: the givers of the feedback, the supervisor who is sharing the feedback and also the person being assessed.
The underlying principle is that no one is perfect and we are all trying to improve upon towards the perfection. The goal of the organisation and the individual here is to improve the individual for a better performance in the organization and to achieve life objectives.

Conditions for Successful 360 Feedback
360 DFB is a powerful activity and an experience in itself as it's created based on certain parameters which gets dissected from the past patterns of an individual based on his understanding and demonstrated behaviors.
We must respect that all individuals are unique and they have untapped potential, and that their understanding of facts may differ based on their past experiences.
The 360 DFB activity is in fact a short moment of truth and one has to experience it, feel it and take it positively.
As a way of aligning one's individual goals with the organizational goals it is very important.
