A simplified, non-threatening and direct version can be used by individual employees to help set their performance targets and measures. It is conducted by the appraisee to the appraisee for their self evaluation and can be anonymous. It simply asks:
- "What would you like me to do more of?"
- "What would you like me to do less of?" and
- "Any other comments".
This is used as one part of a continuous performance and development process.

I agree with Greg. In fact, I advice my clients to use the same method for customer satisfaction surveys, albeit with a few differences.
